## What to know

- SUSTAIN 7 compared specific weekly doses in type 2 diabetes. Its glucose and weight results do not settle every current prescribing choice.

- The studied population, comparator and follow-up determine how far the result travels.

- Use the findings to frame a discussion, rather than predict an individual result.

## A useful comparison with a precise boundary

SUSTAIN 7 supports a fairly specific conclusion: semaglutide produced larger average reductions in blood glucose and body weight than dulaglutide at the paired doses tested in adults with type 2 diabetes. It is direct randomized evidence, which makes it more informative for that question than placing results from unrelated trials side by side. It is also a 2018 diabetes study, rather than a trial of every dose available today or a comparison of obesity treatment programs. Keep the population, dose and outcome attached to the headline. Removing any of those details makes the result sound broader than the experiment actually was.[\[1\]](https://glp1nerds.com/research/semaglutide-dulaglutide-sustain7/#reference-1)

## Who entered the trial

Investigators randomized 1,201 adults whose type 2 diabetes remained inadequately controlled while taking metformin. Follow-up was 40 weeks. The four groups received once-weekly semaglutide at 0.5 or 1.0 mg, or once-weekly dulaglutide at 0.75 or 1.5 mg. The comparison paired the lower doses and the higher doses; it did not establish that equal milligrams represent equal biological exposure. Treatment was open label, so participants and clinicians knew the assigned injection. Randomization improves comparability at the start, while lack of blinding still matters for expectations, symptom reporting and decisions to remain on treatment. Both design features belong in a balanced reading.[\[1\]](https://glp1nerds.com/research/semaglutide-dulaglutide-sustain7/#reference-1)

## What the glucose endpoint means

The main glucose measure was HbA1c, which summarizes blood glucose exposure over time. In the higher-dose pair, average HbA1c fell by 1.8 percentage points with semaglutide and 1.4 points with dulaglutide. The estimated between-group difference was about 0.41 percentage points in semaglutide's favor. That is a difference in percentage points of HbA1c, not a 41% reduction in diabetes risk. It also does not tell an individual how close they will get to their own target. Starting glucose, other treatment, tolerability and follow-up all shape that practical question. The finding is strongest as a comparative average, not a promised laboratory result.[\[1\]](https://glp1nerds.com/research/semaglutide-dulaglutide-sustain7/#reference-1)

## The weight finding is secondary context

In that same higher-dose pair, average weight declined by 6.5 kg with semaglutide and 3.0 kg with dulaglutide over 40 weeks. These are changes within each assigned group, rather than the extra effect of either medicine over no treatment. The estimated difference between them was approximately 3.55 kg. Participants had diabetes and received the trial's follow-up; those circumstances are part of the result. Quoting the larger number as an expected loss for anyone shopping for a weight management subscription would skip both the comparator and the population. A weight outcome can inform a diabetes treatment conversation without turning the study into an obesity indication.[\[1\]](https://glp1nerds.com/research/semaglutide-dulaglutide-sustain7/#reference-1)

## Higher present-day doses change the question

A common mistake is to treat an old direct comparison as a permanent ranking of two molecules. Current Trulicity prescribing information includes adult dosing beyond the 1.5 mg dulaglutide group studied here, with an adult maximum of 4.5 mg weekly. SUSTAIN 7 cannot show what would have happened had that higher dose been randomized against semaglutide. The missing comparison is a real evidence boundary, not proof that the original result is wrong. It means a contemporary discussion should identify the actual product and intended regimen before borrowing the trial's conclusion. This article describes study doses, not instructions to start, increase or exchange medication.[\[3\]](https://glp1nerds.com/research/semaglutide-dulaglutide-sustain7/#reference-3)

## How to read the subgroup follow-up

A later SUSTAIN 7 analysis examined whether results varied with characteristics such as age, sex, diabetes duration, baseline HbA1c and body mass index. Its broad pattern supported semaglutide's comparative efficacy across the groups examined. That is useful reassurance about consistency, but the analysis reuses the original trial participants. It is not a second independent experiment. Smaller subgroups also provide less precise estimates, and a chart split by body size is not a personalized prediction engine. The most defensible use is to ask whether an apparent exception has convincing evidence, rather than assume one subgroup bar can choose a prescription for a particular reader.[\[2\]](https://glp1nerds.com/research/semaglutide-dulaglutide-sustain7/#reference-2)

## Tolerability is part of effectiveness

A glucose or weight advantage has limited practical value when someone cannot continue treatment. Gastrointestinal problems were common with both medicines in SUSTAIN 7 and were a leading reason for treatment discontinuation. The pattern was not a simple declaration that one drug never causes nausea or that the lower milligram number is gentler. Study averages combine people who tolerate treatment well with people who stop it. For a clinical discussion, ask how symptoms will be assessed, how quickly the team responds and what happens if continuing is not appropriate. Those care questions do not erase the efficacy result; they determine how usable it may be.[\[1\]](https://glp1nerds.com/research/semaglutide-dulaglutide-sustain7/#reference-1)

## Safety information comes from more than this trial

Forty weeks in a comparative glucose study cannot resolve every uncommon or delayed harm. Current dulaglutide labeling includes a boxed warning about thyroid C-cell tumors observed in rodents, with uncertain relevance to humans, and contraindications involving personal or family history of medullary thyroid carcinoma or MEN 2. It also addresses pancreatitis, severe gastrointestinal reactions and other precautions. A relatively small difference in an efficacy table does not override contraindications or clinical history. The right safety reference is the current label for the exact product, alongside an individual assessment, rather than a consumer summary of one randomized trial.[\[3\]](https://glp1nerds.com/research/semaglutide-dulaglutide-sustain7/#reference-3)

## Do not substitute a heart outcome claim

HbA1c, kilograms and cardiovascular events answer different questions. SUSTAIN 7 was designed around glycemic comparison; it was not a dedicated event-driven test of which medicine prevents more heart attacks. Dulaglutide's current cardiovascular indication rests on a separate evidence program described in its prescribing information. A reader should therefore keep two evidence folders mentally separate: one for the direct glucose and weight comparison, and another for the relevant outcomes trials and current indications. Combining them into a single score can make a clinically useful medicine look inadequate simply because the chosen endpoint is not the one that matters most for a particular patient.[\[1\]](https://glp1nerds.com/research/semaglutide-dulaglutide-sustain7/#reference-1) [\[3\]](https://glp1nerds.com/research/semaglutide-dulaglutide-sustain7/#reference-3)

## The practical conclusion

The strongest takeaway is that the studied semaglutide doses outperformed the paired dulaglutide doses on average glucose and weight measures in this metformin-treated diabetes population. Novo Nordisk funded the trial, another detail worth retaining when judging the full research record. None of this establishes which online clinic delivers better care, which prescription an insurer will cover or what a particular patient should change. Bring the actual formulation, treatment goal, medical history and budget to a clinician. Then use SUSTAIN 7 as one well-defined piece of evidence. It is valuable precisely because its answer can be stated clearly without pretending that it settles every later question.[\[1\]](https://glp1nerds.com/research/semaglutide-dulaglutide-sustain7/#reference-1)
